Referral, screening and recommendation process

Our Referral, Screening and Recommendations Process will ensure that our services are providing appropriate support, at the right time

We've introduced a new approach to support children, young people, and their networks in gaining a deeper understanding of a young person's needs, behaviours, and what sits beneath this

If you would like to speak to us and are in need of more responsive advice - please see our duty and urgent advice page.

Duty and Urgent Advice

Family held in hands

Whilst this process involves a number of steps and actions from those involved – the benefits from this will include…

  • a therapeutic support plan better informed by the child’s perspective;
  • the child and their network will have a deeper understanding of need and how to respond;
  • important information will be gathered by the service when prioritising resources and access to our interventions.

If you would like to access our Referral, Screening and Recommendation Process on behalf of a child or young person, please first check they are eligible.

  • Their legal status recognises that they are a Child in Care or are a ‘Care Leaver’ of Derby City or Derbyshire.
  • They currently reside within Derbyshire or Derby City.
  • They are aged 0-17 (available now) or 18-24 (available Jan 26)*.

*PLEASE NOTE: The process for those, who are aged 18+ is currently UNDER DEVELOPMENT and will not be available until January 2026.

Print the ‘My Voice’ or ‘My Planet’ booklet – use this as a tool for gathering the child/young person’s perspectives on their emotional wellbeing (see guidance below for hints and advice on how to do this). You can select the booklet that feels most appropriate for the child/young person.

Complete the screening measures as part of the referral – these are intended to help you and us understand trauma symptoms, carer relationships and emotional wellbeing. The required measures are:

  • CRIES-8 (completed by child age 8-17)
  • RCADS – 'Revised Child Anxiety and Depression Scale' (completed by child age 8-17)
  • TAYC – 'Thinking About Your Child' (carer completed)
  • SDQ – 'Strengths and Difficulties Questionnaire' (if completed in last 3 months, this can be used)

Complete the referral form, containing important information about the young person’s circumstances. Then email the completed referral pack to [email protected]

The service will clinically review all referral packs received and present these within Prioritisation Panel where the outcome will be agreed with commissioners, which could include:

  • The offer of a Detailed Consultation – a one off or first intervention for CYP's Therapeutic Parenting Team (network)
  • An outcome summary and analysis of the referral pack.
  • Other recommendations to meet the CYP's needs.

Please find all documents and tools you will need below available to download, with detailed guidance and instructions.

The CRIES-8 and the RCADS have both been translated into a wide range of different languages for young people where English is not their first language. See the links below for the translated versions.
