Register and update your information
Before you register:
Bucks Activity Project works with children and young people with varying levels of disability.
Your child must:
• Be under 19 years of age and live in Buckinghamshire
• Identified as having a disability with additional needs
• Require support to access play and leisure activities
If your child has an Education, Health and care Plan (EHCP) but has not got a recognised disability you can still register with Bucks Activity Project.
Confirmation of disability or SEN may be required from a health, social care or education professional.
This could include:
• GP
• consultant
• community nurse
• school or social worker.
Definition of “disability” under the Equality Act 2010.
In the Act, a person has a disability if:
• They have a physical or mental impairment
• The impairment has a substantial and long term adverse effect on their ability to perform normal day-to-day activities.
For the purposes of the Act, these words have the following meanings:
• “substantial” means more than minor or trivial
• “long term” means that the effect of the impairment has lasted or is likely to last for at least twelve months
• “normal day-to-day activities’ include everyday things like, washing, walking and going shopping.
Some families may need additional support to access our services. If English is not a family's first language and you need communication support or financial assistance we can offer additional help.

Do you struggle to read and write?
If you are struggling to complete the forms or understand the service we are able to help. Please contact us. We can either arrange for your social worker to support you or come visit you to complete the paperwork. We can then make all contacts with the service to arrange access to our services over the phone.

- Working Tax Credit (NOT Child Tax Credit)
- Jobseekers Allowance
- Council Tax Benefits
- Pension Credit
- Attendance Allowance
- Employment Support Allowance
- Incapacity Benefit
- Income Support
If you do, you are entitled to reduced rate activity fees. You must provide yearly proof of the benefits to continue to receive the reduced fees
On registering, you will be asked to complete a support questionnaire to help us understand what level of short breaks service your child should be offered for the year.
This might be a standard offer or enhanced offer with activities available from Bucks Activity Project or one of our sub-contracted partners.
If your child will achieve better outcomes by accessing universal services, we will support you in identifying suitable activities and helping your child to access these.
If your child has a social worker, we may ask your social worker to help us understand your family needs.

About these forms:
A support questionnaire needs to be completed for all children registered with Bucks Activity Project. The questionnaire determines the level of offer available to each individual child. You only need to complete one questionnaire, which depends on your child's age. Our service supports children up until their 19th birthday.

This form needs to be completed by a medical professionals to advise the team on how to administer your child's emergency prescribed medication (PRN). This normally includes Epilepsy and Anaphylaxis emergency medication. This form needs to be updated on an annual basis, however if there is no change to the protocol a new signature can be added to the same protocol. For asthma inhalers and over the counter medication, we do not need this completed.
