Bucks Activity Project works with children and young people with varying levels of disability.

Your child must:
• Be under 19 years of age and live in Buckinghamshire
• Identified as having a disability with additional needs
• Require support to access play and leisure activities

If your child has an Education, Health and care Plan (EHCP) but has not got a recognised disability you can still register with Bucks Activity Project.

Confirmation of disability or SEN may be required from a health, social care or education professional.

This could include:
• GP
• consultant
• community nurse
• school or social worker.

Definition of “disability” under the Equality Act 2010.

In the Act, a person has a disability if:
• They have a physical or mental impairment
• The impairment has a substantial and long term adverse effect on their ability to perform normal day-to-day activities.

For the purposes of the Act, these words have the following meanings:
• “substantial” means more than minor or trivial
• “long term” means that the effect of the impairment has lasted or is likely to last for at least twelve months
• “normal day-to-day activities’ include everyday things like, washing, walking and going shopping.

Some families may need extra support to access our services. If English is not your first language, or if you need help with communication, paperwork, or financial assistance, we are here to help.

• Understand how the service works
• Complete the registration process
• Access sessions confidently

Do you struggle to read or write?

If you find it difficult to complete forms or understand written information, we can help.
We can:

• Arrange for your social worker to support you, or
• Visit you to complete the paperwork together

Once registered, we can make all contact with the service on your behalf over the phone.

Do you receive any of the following benefits?

• Housing Benefit
• Working Tax Credit (not Child Tax Credit)
• Jobseeker’s Allowance
• Council Tax Benefit
• Pension Credit
• Attendance Allowance
• Employment Support Allowance
• Incapacity Benefit
• Income Support

If you receive any of these, you are entitled to reduced‑rate activity fees.
You will need to provide yearly proof of your benefits to continue receiving the reduced rate.

When you register, you’ll be asked to complete a support questionnaire. This helps us understand the level of short breaks service your child should be offered for the year, delivered by Bucks Activity Project or one of our sub‑contracted partners.

Depending on your child’s needs, this may be:

• A standard offer (36 hours)
• An enhanced offer (90 hours)

If we feel your child would achieve better outcomes by accessing universal services, we will support you to identify suitable activities and help your child access them.

If your child has a social worker, we may contact them to help us understand your family’s needs more fully.

About these forms:

A support questionnaire needs to be completed for all children registered with Bucks Activity Project. The questionnaire determines the level of offer available to each individual child. You only need to complete one questionnaire, which depends on your child's age. Our service supports children up until their 19th birthday.

Bucks Activity Project Medication consent form

About this form:

If your child requires emergency medication (such as an asthma inhaler or Buccal Midazolam) or any medication to be taken during a session, we must have the correct medication consent and all appropriate forms completed in advance.

Without completed consent and the required documentation, we are unable to administer any medication to your child. This may also restrict your child’s ability to attend sessions, as we must ensure their safety at all times.

Bucks Activity Project Emergency Medication Protocol (Procedure plan for epilepsy emergency medication)

About this form:

This form needs to be completed by a medical professionals to advise the team on how to administer your child's emergency prescribed medication (PRN). This normally includes Epilepsy and Anaphylaxis emergency medication. This form needs to be updated on an yearly basis, however if there is no change to the protocol a new signature can be added to the same protocol. For asthma inhalers and over the counter medication, we do not need this completed.

Gastro feeding consent form

About this form:

This form must be completed by a medical professional (such as a Dietician) to advise our team on how to safely administer your child’s feed, water, or medication. The protocol must be updated every year. If there are no changes, a new signature can be added to the existing protocol to confirm it is still accurate.
