This page is for practitioners working with children and young people with disabilities and parents or carers who want to self refer to access our daytime and evening activities. Learn who is eligible for the activities and how to make a referral.

Daytime & Evening Activity Eligibility

Children and young people aged 5 to 17 who have a disability, does not have to be diagnosed. Disabilities could be a learning disability, autism, ADHD, global developmental delay, complex medical needs and more. Typically the children and young people coming along to our services will not have the opportunity to take part in any out of school activities. This means that they and their parents are not getting a break from each other. We can either help try and find community activities or they can come along to our activities.

The form will be sent through to our referral inbox and our team will review all referrals received on a weekly basis and will get back to you to discuss the referral further and next steps
